Integrating family Medicine and Pharmacy to Advance primary Care Therapeutics

IMPACT is a large-scale demonstration project supported by the Ontario Primary Health Care Transition Fund (2004-2006). IMPACT aims to improve drug therapy using a collaborative care model that integrates pharmacists into the primary health care team. The pharmacists' main service will be individual patient assessments to identify, prevent or resolve drug-related problems. Quantitative and qualitative methods will be used to evaluate the process of integration, pharmacist service uptake, drug-related patient outcomes, and the costs associated with program set up and implementation for sustainability.

IMPACT involves a team of over 20 clinicians and researchers and is being led by Lisa Dolovich, Janusz Kaczorowski, and Connie Sellors from McMaster University, Kevin Pottie and Barbara Farrell from the University of Ottawa, and Zubin Austin from the University of Toronto.
